The Hidden Spaces: Why Brushing Your Teeth Is Only Half the Job
Imagine washing your car but completely ignoring the wheels and the undercarriage. You spend hours polishing the bonnet and the doors, making them shine brilliantly. From a distance, the car looks perfectly clean. However, the hidden areas remain covered in corrosive mud and grit. Eventually, that hidden grit causes the wheels to seize and the frame to rust.
This analogy perfectly describes how most people clean their teeth. They diligently scrub the front, back, and biting surfaces of their teeth with a toothbrush. They look in the mirror, see a bright smile, and assume the job is done.
Unfortunately, they have completely ignored the hidden spaces. Brushing only cleans roughly sixty percent of the surface area of a tooth. The remaining forty percent lies in the tight contact points where the teeth touch each other. Let us explore the fascinating, often misunderstood world of interdental cleaning, and explain exactly why flossing is non-negotiable for a happy dental future.
The Anatomy of a Tooth Contact Point
To understand why brushing fails to clean the whole tooth, you must look at the physical shape of your teeth. Your teeth do not sit perfectly flat against one another like bricks in a wall. They are curved and contoured.
They touch each other in a tiny area called the contact point. Below this contact point, the teeth taper inwards toward the gum line, creating a small, triangular space. Dentists call this space the interproximal embrasure.
Your gum tissue naturally fills this triangular space, forming a small peak called the interdental papilla. This creates an incredibly tight, dark, and protected environment. The bristles of a standard toothbrush are physically too thick to penetrate past the tight contact point and reach down into this triangular space.
The Breeding Ground for Biofilm
Because toothbrush bristles cannot reach the interproximal spaces, these areas become the perfect breeding ground for plaque biofilm.
Plaque is not just leftover food. It is a highly organised, living colony of bacteria. When you eat, microscopic food particles and sugars wash into the spaces between your teeth. The bacteria in the plaque feast on these trapped nutrients. Because the area is completely protected from your toothbrush and your tongue, the bacterial colony grows rapidly undisturbed.
As the colony grows, the bacteria excrete lactic acid. Because the space is so tight, your saliva struggles to flow through and neutralise the acid. The acid sits directly against the side of the tooth, silently dissolving the enamel.
This process causes interproximal cavities. These cavities are incredibly dangerous because they remain completely invisible to the naked eye. They start between the teeth and tunnel deep into the dentine. You will not see or feel an interproximal cavity until it reaches the nerve and causes severe pain, or until the top of the tooth physically caves in.
The Catalyst for Periodontal Disease
Cavities only represent half the danger. The undisturbed bacteria between your teeth also trigger a severe inflammatory response in your gums.
When plaque sits against the interdental papilla (the gum peak between the teeth), your immune system recognises a bacterial invasion. It sends white blood cells to the area to fight the infection. This causes the gum tissue to swell, turn red, and become highly sensitive.
This is gingivitis. The most obvious sign of gingivitis is bleeding gums. If your gums bleed when you brush or floss, you have an active bacterial infection.
If you do not physically remove the plaque between the teeth, the gingivitis progresses into periodontitis. The chronic inflammation causes the bone supporting the teeth to literally melt away. The gums detach from the tooth, creating deep infected pockets. Eventually, the teeth become loose and fall out. Periodontal disease almost always starts in the uncleaned spaces between the teeth.
Breaking the Biofilm Mechanically
You cannot kill plaque biofilm with mouthwash alone. The biofilm produces a protective, slimy outer layer that repels liquid rinses. You must physically disrupt the colony using mechanical friction.
This is the entire purpose of interdental cleaning. When you slide a piece of floss between your teeth, you physically scrape the sticky bacterial colony off the enamel. You break up the organised structure of the biofilm, rendering it harmless.
Choosing Your Interdental Weapon
Many people hate traditional string floss. If you find string floss difficult to wrap around your fingers, or if it constantly snaps between tight teeth, you have other excellent options.
Interdental brushes look like tiny, wire bottle brushes. They come in various sizes to fit different spaces. You simply slide them back and forth between the teeth near the gum line. Research shows that for people with slight gum recession, interdental brushes clean better than string floss.
Floss picks hold a small piece of string on a plastic handle, making it much easier to reach the back molars using just one hand. Water flossers use a high-pressure, pulsating stream of water to blast the plaque out from between the teeth. Water flossers are fantastic for people with braces, bridges, or dental implants.
Making the Habit Stick
The hardest part of interdental cleaning is building the daily habit. Start small. Do not promise to floss your entire mouth every single day if you currently never floss.
Commit to flossing just your bottom front teeth every night for a week. The task takes ten seconds. Once you build the muscle memory and the habit sticks, slowly add more teeth.
